IRMAA brackets & surcharges — official tables

These are the complete CMS tables, verified October 2026 against the CMS fact sheets. MAGI for IRMAA = your adjusted gross income plus tax-exempt interest. Premiums are per person, per month. Your premium year is priced off the tax return from two years earlier: 2026 premiums use 2024 MAGI; 2025 premiums used 2023 MAGI.

Married filing separately — read this first: the MFS table has no gentle tiers. One dollar over the first threshold jumps straight to near-top rates. It is the single most expensive checkbox in Medicare pricing.

2027 brackets

Not yet published. As of October 2026, CMS has released no 2027 premium fact sheet; the tables are expected in November 2026, following CMS's pattern. Anyone quoting exact 2027 IRMAA amounts today is projecting, not reporting. Our 2027 page explains what to do while you wait.

Sources: CMS, "2026 Medicare Parts A & B Premiums and Deductibles" (Nov 14, 2025) and the 2025 edition (Nov 8, 2024); cross-checked against SSA's Medicare Premiums page. If a figure here ever disagrees with your notice, your notice controls.
