Form SSA-44, explained field by field

Form SSA-44 — "Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event" — is how you ask Social Security to price your Medicare premiums off a more recent year instead of the tax return from two years ago. It is a request for a new determination, not an appeal, and that distinction runs the whole process: there is no 60-day deadline, but the reduction is anchored to the year you file.

Step 1 — the event (where cases are won and lost)

You check the category that fits: death of a spouse; marriage; divorce or annulment; work stoppage or work reduction; loss of income-producing property beyond your control; loss of pension income; or an employer settlement payment (for example, from a former employer's bankruptcy). If your situation isn't one of these, the form cannot help you — no matter how much your income fell. Our events guide works through the borderline cases.

Step 2 — the two incomes

The form asks for the MAGI from the tax year Social Security used, and your estimated MAGI for the current year. MAGI here means adjusted gross income plus tax-exempt interest — the most common arithmetic error on the form is forgetting the tax-exempt interest line. Estimate honestly and document how you built the number: when the IRS later sends Social Security your actual return, the determination is reconciled, and an estimate that was really a wish can be corrected retroactively.

Step 3 — the evidence

Each event has its own proof: a retirement or separation letter for work stoppage, a divorce decree, a death certificate, pension termination documentation, insurance or court records for lost property. Attach copies, never originals. Thin evidence is the leading cause of denials — the packet matters more than the form.

Filing it

After you file

If approved, the new premium generally applies from January 1 of the year you made the request — file in the same calendar year as the reduction whenever you can. You keep paying the current premium until the decision lands. If you're denied, the formal ladder opens with reconsideration (Form SSA-561-U2) inside 60 days — see the denial guide.
